The McKesson Telehealth Advisor™ disease management monitoring solution helps patients become active participants in their care by answering diagnosis-specific questions on an electronic appliance daily, while receiving targeted patient education about their condition in order to modify behavior for improved outcomes. The solution consists of three components: disease management programs, Health Buddy® Appliance, from Health Hero Network, and Monitoring Application.

Benefits

Improves disease management and patient monitoring

Improves patient education and increases patient involvement

Allows efficient resource management and reduces healthcare costs

Features

McKesson Telehealth Advisor™ is comprised of three components:

Disease management programs: The copyrighted disease management programs feature more than 10,000 dialogues, which are based on appropriate standard practice guidelines. McKesson has a growing library of disease management programs that allow providers to reach a broad base of patients and patient education needs. Examples of these programs include diabetes, asthma and heart failure.

Health Buddy® Appliance: An in-home patient communication appliance, the Health Buddy Appliance was awarded Best Product by Business Week and Best Enabling Tool by the Disease Management Association of America. This system collects data about the patient’s chronic condition and communicates diagnosis-specific information to the patient on a daily basis.

Monitoring Application: The Internet-enabled patient monitoring application allows the healthcare provider to access updated patient information anytime, anywhere. The application helps care providers quickly identify at-risk patients using color-coded risk levels that are based on symptoms, patient behaviors and self-care knowledge.
